Job overview

An opportunity has arisen within the CNWL NHS Foundation Trust, Community Health Services, for a Rehabilitation Assistant to join our Staying Steady, falls prevention service. Due to the expanding and innovating project work, currently under development, we are further resourcing our team to carry out this exciting new work.   Based in Bletchley, Milton Keynes, Staying Steady is a multi-professional team who undertake multifactorial risk assessments, provide enabling solutions using equipment prescription, advice, signposting and other treatment interventions to reduce falls risks for adults in Milton Keynes. 

This post will provide an opportunity to further develop your assessment and treatment skills for people who have fallen or are at risk of falling.   You will be working alongside experienced and specialist Physiotherapists, Occupational Therapists, Nurses, Rehabilitation Support Workers and Consultant, to provide coordinated assessment and goal orientated intervention to service users.

Main duties of the job

  • To undertake specific skilled support work to aid rehabilitation of service users, in achievement of optimal independence with or without the assistance of others
  • To provide flexible programmes and interventions of therapeutic activities, under the direction of Home 1st health professionals.
  • Follow treatment plans, developed by health professionals in the service.
  • Work without direct supervision, monitoring progress and provide feedback to health professionals on outcomes

Detailed job description and main responsibilities

  • Under the guidance of a Health professional, carry out interventions in the community, predominantly patients own home
  • Participate in competency framework developing skills in treatment planning
  • To provide high standards of documentation and communication, including the use of electronic systems
  • Support colleagues by assisting in the provision of group therapy exercise and education sessions
  • To work autonomously within the recognised boundaries of the remit of the rehabilitation assistant
  • To work predominately alone in a community setting, escalating the management of patients who have fallen; either on an individual basis or in a group setting.
  • To undertake falls prevention home environment assessments using knowledge and skills acquired through practical experience and specific training 
  • To discuss with the patient individual functional goals and support the patient to carry out agreed plans
  • To support the delegated tasks in organising and managing appointments for people requiring group exercise sessions, provide telephone review of patients attending groups
  • Working alone to support treatment plans.  This includes but is not limited to: Practice of mobility / Gait re-education / home exercise programme / practical activities involving personal care, kitchen activities and social/leisure activities
  • To prescribe, order and provide appropriate equipment and/or to refer on as necessary e.g. social services, Medequip, Age UK.  To liaise with health professionals to order specialist equipment
  • To implement and evaluate the intervention for the patient and where necessary modify or progress the programme according to any change in the patient’s condition under the guidance of the health professional
  • To record and feedback patient’s progress, problems and any changes in clinical status
  • To ensure safety and maximise functional rehabilitation of patients in their home
  • To adapt treatment programmes to suit the patient’s home environment using creative thinking and problem-solving skills
  • To maintain accurate and up to date patient records in a manner that meets the standards required by CNWL
  • To provide advice, support and education to patients, carers, families and professional colleagues regarding treatment
  • To be aware of groups and publications which may be of benefit to the patient e.g. self-help groups and leaflets about falls prevention
  • To plan and work in conjunction with other agencies e.g. Social Services, other Primary and Home 1st Care Teams and Voluntary Agencies to assist with or continue the rehabilitation of patients

Qualifications

Essential criteria
  • Educated to GCSE standard
  • Completion of the Care certificate (or willing to work towards)
  • Full & current UK driving license & provision of vehicle

Experience

Essential criteria
  • Work experience in health and social care setting preferably understanding of rehabilitation and MDT working
  • Therapy background in Community with transferable skills
  • Autonomous working
Desirable criteria
  • Experience of providing group exercise classes and education sessions

Knowledge

Essential criteria
  • Understands & promotes equality, diversity and human rights

Key Skills/Abilities
